Private Pay
Flexible private payment arrangements utilizing personal funds, long-term care insurance, or retirement savings.
Elderly Waiver
Minnesota state waiver program support designed for seniors who require services to continue living in the community.
CADI & BI Waiver
Community Access for Disability Inclusion (CADI) and Brain Injury (BI) programs offering critical support for health-related requirements.
Admission Process
At Imperial Care LLC, we strive to make the admission process simple, timely, and family-centered. Our goal is to ensure every prospective resident receives the appropriate level of care in a safe and welcoming environment.
Submit an Intake Inquiry
A prospective resident, family member, hospital discharge planner, social worker, or care manager submits an intake inquiry with the resident's basic information and care needs.
Clinical Review
Our registered nurse carefully reviews the prospective resident's medical history, current diagnoses, medications, functional abilities, and overall care needs to determine whether our services are an appropriate match.
Initial Response
Within one to two business days, we provide a prompt and honest response regarding our ability to safely meet the prospective resident's needs.
Pre-Admission Assessment
If appropriate, our intake nurse schedules a comprehensive pre-admission assessment to evaluate the resident's physical, cognitive, behavioral, and personal care requirements.
Home Tour
The prospective resident and family are invited to tour Imperial Care LLC, meet our staff, view the living accommodations, and learn about our services and daily routines.
Financial & Admission Documentation
We work with the resident, family, or referral source to ensure all required admission paperwork is completed and funding sources, including private pay, waiver programs, or other payer sources, are active or in progress.
Admission Approval & Move-In
Once the assessment is complete, documentation is approved, financial arrangements are confirmed, and the prospective resident chooses Imperial Care LLC, a move-in date is scheduled. Our team coordinates a smooth transition and welcomes the resident into their new home.
